Day Services
Portsmouth Day Service offers a range of services for adults with learning disabilities, including support for some evening and weekend activities. Staff are based in various locations around the city so that people are able to use their local communities. Everyone is given a key-worker who can offer counselling and will help the individual plan their service, respecting cultural and other needs. The service provides important respite for carers. Access to Portsmouth Day Service is by an assessment of needs; the service is free although there is a charge for lunch. The Horizon Centre aims to create opportunities that enable people with a physical disability to develop their lifestyle, in partnership with their carers, through assessment, informed choices, purposeful activities and respite care. It offers help with occupational therapy aids, accessing other resources, maintaining mobility, using the local community, communication skills, budgeting and shopping skills, cookery skills and leisure interests. Referral should be made through the Duty Officer for Physical Disability Services at Medina House (as above).
Adult Placement is a resource that has been around for many years; at its most basic it is the provision of accommodation and support in an ordinary family home for someone who needs support.
Adult Placement Schemes provide a wide range of flexible services ranging from day care to long-term placements to vulnerable adults and older people.
Adult Placement Carers can offer accommodation in their own home to a maximum of three individuals who have been referred and assessed by a care manager.
Who provides it?
Adult Placement carers are recruited by the Portsmouth AP Scheme, which is managed by Portsmouth City Council. Adult Placement Carers come from all walks of life and range from single carers to families with children, they all live within the Portsmouth area. They are assessed by Adult Placement Officers and approved by a panel of professionals, carers and service users. All carers and family members over 16 years have been checked with the Criminal Records Bureau. The Adult Placement Team monitors and supports carers on a regular basis. Who Pays?
Service users will be required to pay a contribution towards the Adult Placement Carers fee. An officer from the Financial and Benefits Team will visit to complete a financial assessment, from this they calculate the amount you will be asked to pay towards your care.
Older Persons Day Care
Day Care has taken a step forward!
Day Care or Day Opportunities is a chance for people to make new friends, try new things, have fun and generally have a nice day out ~ a meeting place. It is much more than sitting around playing bingo and making craft items. Day trips are also offered and have included in the past boat trips, band concerts, visits to the D-Day museum, and much more.
Specialist days incorporate high staff numbers and smaller client numbers, where special attention is given to personal care and dietary requirements. Other facilities on offer are hairdressing, foot care and advice ~ putting people in touch with what is available in the community to help them. So sit back and enjoy the company of others while enjoying a lovely meal. Some of the Centres are able to provide extended day times to fit in with the Carer's employment or family responsibilities.
It provides some "time out" for Carers to catch up, attend medical appointments or just watch TV without any interruptions.
